sign a photo release giving permission to Weddings by Jerry to use their photographic likeness

for promotional use for Weddings by Jerry. (the images will NOT be sold or given to any other companies)

You will receive an additional $200 Add-On service credit.

For the Budget package -

$0 credit included with the package plus the additional $200 = $200 in total credit

For the Standard package  -

$600  credit included with package plus the Additional $200 = $800 in total credit

For the Deluxe package  -

Choice A - 12 x 12  complete custom album ($1300 value)  plus the Additional $200

Choice B - $1000 credit included with package plus the Additional $200 = $1200 in total credit

*Releases must be received within 30 days of your wedding to qualify,

valid for credit only - no cash value
